Waiver & Release of Liability
THIS AGREEMENT CONTAINS A WAIVER & RELEASE OF LIABILITY AND INDEMNITY AGREEMENT TO WHICH YOU WILL BE BOUND AND UNDER WHICH YOU WILL BE WAIVING IMPORTANT LEGAL RIGHTS. DO NOT SIGN THIS AGREEMENT BEFORE YOU READ THE TERMS AND CONDITIONS.
I, the undersigned, understand that this healing session is not a substitute for medical attention, examination, diagnosis, or treatment. I understand I should consult a physician before beginning any physical program, including sound healing.
I recognize that I am responsible for informing my practitioner of any serious illness or medical condition, including pregnancy, cardiac pacemaker, metal implants in the body, or injury, before every session.
I understand that if I feel discomfort at any time during the session, I am welcome to gently change my position, leave the room, or let the practitioner know.
I understand that any pictures, audio, or visual recordings taken of me in connection with the event can be used for publication, promotion, articles, shows, and advertisement without additional consent and without compensation at this time or any other time.
I accept that neither Alchemy for the Soul LLC, Natasha Dominguez, nor the hosting facility is liable for any injury or damages to persons or property resulting from attending these sessions. This protection includes but is not limited to all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees, and other participants connected with our events.
This waiver and release agreement is intended to be as broad as is allowed under the applicable law and applies to any and all claims for damages.
